Report an incident to ASI Incidents range from generic observations and minor concerns to major problems perceived by users and stakeholders of the certification systems that ASI works with. Once an incident is reported, it is assessed and prioritized by ASI to determine if and when a response is needed. Some incident may require immediate follow up by ASI while others can be addressed during normally scheduled audits. ASI may also determine that no follow up is needed. Whatever the response, all incidents reported to ASI are logged for future reference. We consider any incident important in that it may support our understanding of how to safeguard the integrity of social and environmental standards by providing credible and independent assurance. Incidents differ from complaints in that the reporter can be anonymous and the incident does not first have to have been addressed by the conformity assessment body (CAB) and/or certificate holder in order for ASI to address it. If you have been through a formal dispute resolution process with a CAB already and wish to bring the issue to ASI, you should file a formal Complaint and not report it as an incident.

How to report an incident When you report an incident to ASI, please provide as much detail as possible regarding your concerns. Some good information to include could be:

  • the 5 W’s: who, what, when, where, why
  • any risks you perceive from this issue
  • the history of the issue, if any
  • any evidence to substantiate your claims
